The Milford Hospital Case Management and Social Services staff provide the information, resources and assistance needed by our patients and their families to ensure that their needs are being met. Staff will work with you and your family in coordinating a plan of care for your hospital stay and your discharge.
Case managers are specially trained nurses who serve as patient liaisons providing information and support for patients and their families. They help in coordinating and reviewing the care plan for patients.
Social workers assist patient and their families in developing a comprehensive and effective discharge plan by assessing the individual medical and psychosocial needs of each patient. Discharge plans may include: home health care services, outpatient rehabilitation services (physical therapy, occupational therapy, and speech therapy), short-term rehabilitation, sub-acute care and skilled nursing facility.
Planning for any needed after-care or services should begin early in your hospital stay to assure that you have a safe and appropriate discharge plan. Also, discharge plans need to be coordinated with your health insurance, as some services may not be covered.
Social workers and nurse case managers are available 7 days a week to answer questions, consult with family members, offer counseling and make community referrals as needed.
